This document, “Note to the Medical Chart,” written by Hugh Calkins, reveals that trainee Richard Wu was operating a catheter when it was placed in the wrong heart chamber. Johns Hopkins maintains that this is a “Privileged Document” and wants it returned.
The note, which had been withheld during the course of four years of requests for the medical record, was finally released by accident in June 2006. The fact that Hopkins desperately wants it back would seem to indicate that the hospital never intended for Pam to know that Hugh Calkins had not performed the procedure.
Among the many misstatements is this: “The procedure was performed in an attempt to avoid cardiac surgery.”
